  • Offers timely access to primary medical care through a resident Nurse and visiting Physician
  • Provides access to other health care professionals outside of the clinic
  • Coordinates documentation if patient receives care from a member of the team
  • Provides diabetes case management
  • Provides blood pressure checks
  • Offers mental health counselling through a visiting mental health counsellor from Sioux Lookout on a regular basis
    • Offers primary health care including but not limited to:
    • Chronic disease management
    • Health promotion
    • Disease prevention
    • Nutritional counselling

    Frais: Service - Some tests and/or forms are not covered by Ontario Health Insurance Plan (OHIP); None for most services
