Description of Services:	Offers timely access to primary medical care through a resident Nurse and visiting Physician Provides access to other health care professionals outside of the clinic Coordinates documentation if patient receives care from a member of the team Provides diabetes case management Provides blood pressure checks Offers mental health counselling through a visiting mental health counsellor from Sioux Lookout on a regular basis Offers primary health care including but not limited to: Chronic disease management Health promotion Disease prevention Nutritional counselling

Fees:	Service - Some tests and/or forms are not covered by Ontario Health Insurance Plan (OHIP); None for most services
