Description of Services: Range of services for children and adults with arthritis * the services provided by Occupational Therapists, Physiotherapists and Social Workers include:
  • consultation and guidance on arthritis self -management in the community /home/workplace , including physical, emotional and psychological management strategies as required
  • customized arthritis service plan
  • group education that includes information and management techniques for arthritis

Services are provided individually (in person, by telephone, or videoconferencing) or in a virtual group setting
Requirements
Fees: Free to Ontario residents with a valid OHIP card
Eligibility - Population(s) Served: Ontario residents of all ages (including children) with a suspected or confirmed arthritis diagnosis * clients are matched on registration to the closest available site to their home or virtually
Application: Self-referral or referral from a health care provider. Call or download referral form
