Description of Services: THP is integrating all its inpatient rehabilitation services to a designated space at the University Health Network Reactivation Care Centre (THP-UHN RCC) * THP’s rehab services is provided in one location to provide high quality, coordinated care in an environment designed to enable recovery * the program will relocate to its permanent home and centre of specialized care at the Gilgan Family Queensway Health Centre, scheduled to open in 2029
    • integrated rehabilitation team provides specialized services to boost patients’ cognition, strength, and endurance, including occupational therapy, physiotherapy and recreational therapy
    • access to specialized equipment required for individual therapy plans
    • free daily shuttle to and from Credit Valley Hospital from 6am to 7:45pm
    • care team includes physicians, physiatrists, nurses, nurse practitioners, physiotherapists and assistants, occupational therapists and assistants, speecl language pathologists, recreational therapists, social workers and discharge planners, registered dietitians, pharmacists
