Description of Services: Provides students with an intellectual disability, a transition plan that is person-centered and used to facilitate a smooth and successful transition from school to community life * transition coordinators can assist high school students and their families to plan for their future and to set personal goals.

Services provided:
  • facilitates transition from school to community life through a self-directed transition plan
  • coordinates plans to support personal goals
  • provides opportunities to build connections within the community
  • assists with applications for funding and referrals to community services
  • collaborate and consult with local high schools

Eligibility - Population(s) Served: Adults and youth 14 to 21 years of age, with an intellectual disability attending a local high school
