Organization: Mountdale Family Health Clinic
Description of Services: Medical care team focused on family and community health
  • Offers timely access to high quality primary care
  • Provides access/referrals to other health care professionals within and outside of the Medical Associates and Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
  • Provides a Walk-in clinic
Offers primary health care including but not limited to:
  • Chronic disease management
