| Description of Services: | A multidisciplinary team of mental health professionals offering intensive support to individuals with complex mental health and physical health needs. Services include addictions, behaviour therapy, case management, nursing and psychiatry. | ||

| Eligibility - Population(s) Served: | Homelessness / At Risk Only | ||
| Application: | Referrals are through The Access Point. Referrals can be made by physicians, hospital professionals, mental health and community organizations. Self-referrals and referrals by family members are also accepted. Please call or complete the online application form. Eligibility criteria include: - history of serious mental health problems with impaired functioning in the community - inability to consistently perform activities of daily living - inability to consistently maintain a safe living situation and multiple recent hospitalizations and/or use of hospital emergency departments. Also, a minimum of three of the following: - inability to meet basic survival needs, current homelessness or precarious housing - criminal justice involvement in the context of mental health and addictions problems in the past year; - coexisting substance use disorder; - developmental delay or acquired brain injury; - acute and/or chronic medical illness which requires intensive support and difficulty effectively utilizing traditional office-based out-patient services. | ||
