Description of Services: Responsible for Medical Transportation assistance and processing Non-Insured Health Service claims for community and Band members
  • Submits medical appointment travel, mileage, meal and accommodation requests on behalf of individual
  • Assists with making travel arrangements
  • Maintains client documentation, receipts and related information
  • Provides transportation to medical appointments within the community and to medical/specialist appointments in Dryden, Winnipeg and Thunder Bay

Eligibility - Population(s) Served: Members of Eagle Lake First Nation who require:

  • assistance with Non-insured Health Benefits (NIHB) arrangements and forms, and/or

  • assistance with medical transportation due to age, illness or disability
Application: Medical Transportation

  • Requires written proof of medical appointment

  • Requires 24 hour advance booking; exceptions made for emergencies

  • Advance cancellation recommended
